Welcome to the forum
Rick Holland,
All forms of Desktop Alert should always display on top (foreground) in all cases including in full-screen mode as long as you have POP Peeper set to "Allow all notifications" in the "Detect Full-screen Apps on new mail event" in "Tools" > "PPTweaker..." > "Misc2" (top option).
What version of POP Peeper are you using?
What Operating System are you using?
Are you using the default Desktop Alert ("Tools" > "Options" > "Notification", option: "When I receive new mail" option set to "Display Desktop Alert" in the "Visual" settings area of the "Notification" page)?
Can you recall any changes you have made to your computer since the last time the Desktop Alert had been displayed as expected (on top)?

I just now tested for this and as long as I have POP Peeper set to "Allow all notifications" in the "Detect Full-screen Apps on new mail event" in "Tools" > "PPTweaker..." > "Misc2" (as mentioned before, which I assume you are using because if you were not you would not have observed the Desk Top Alert after exiting Full-Screen mode) then I do get the Desk Top Alert displayed as expected in Full-Screen mode (see screen-shot below):
(I personally use "Sound only; No Popups" for the "Detect Full-screen Apps on new mail event" setting so I usually hear only sound but changed the setting to "Allow all notifications" for the test which worked as expected as illustrated above).
Keep in mind if you have the POP Peeper interface open then the Desk Top Alert will not display (which makes sense), so if you happen to be viewing a YouTube video in full-screen mode you will hear the audio (sound of alert) but will not get the Desk Top Alert displayed. However this is not what is occurring in your case because you have said that you find the Desk Top Alert displayed after you exit Full-Screen.
Your results are very odd as I am unable to duplicate the problem you are having since the Desk Top Alert does display as expected in full-screen mode in my case on Windows 7 using the current POP Peeper release.
Not that this should make any difference (because it should not) but do you happen to be using a Skin Notifier?
